GG0130 Self-Care Items
Self-care items are where OT documentation carries Section GG. Each item is coded on the standard 06 to 01 scale based on usual performance. The item set below reflects SNF MDS reporting; IRF, LTCH, and home health report overlapping but not identical sets.

Item letters and definitions per SNF MDS reporting. Verify against the current RAI Manual Chapter 3 before clinical use; there is no item D in this set.

Coding notes
- Eating includes modified food consistencies; tube feeding status routes to a not-attempted code discussion, not a low performance score by default.
- Oral hygiene includes denture management: removing, replacing, and managing soaking and rinsing equipment.
- Toilet hygiene is the clothing management and perineal hygiene around toileting, distinct from the toilet transfer, which is a mobility item.
- Shower/bathe self excludes the transfer in and out; washing, rinsing, and drying code here, the transfer codes under mobility.
- Upper and lower body dressing split at the waist; footwear is its own item and includes socks, shoes, and orthoses.
