Ten activities of daily living scored by level of assistance needed, totaling 0 to 100 with higher scores indicating greater independence. Scoring weights mobility and continence more heavily. Published interpretation cutoffs vary between authors, so report the raw score and item pattern rather than a severity label alone, and compare admission to discharge on the same version of the index.
